This significant National Reform is founded on the principles of citizenship, self determination and choice for service users.

BEHAVIOUR MANAGEMENT CONSULTING (BMC):
Behaviour support is about creating individualised strategies for people with disability that are responsive to the person’s needs, in a way that reduces and eliminates the need for the use of regulated restrictive practices.
Behaviour support focuses on evidence-based strategies and person-centred supports that address the needs of the person with disability and the underlying causes of behaviours of concern, while safeguarding the dignity and quality of life of people with disability who require specialist behaviour support.
- The Health & Community Solutions Team provide expert Behavior Management Consultation support within the Company’s framework of consumer directed care, working with individuals with disability, their carers and family to achieve meaningful and sustainable behavioral outcomes.
SUPPORT COORDINATION (SC) / CASE MANAGEMENT
- Service Navigation and Resourcing
- Service Planning and Implementation
- Referral, Establishment and Monitoring of supports
- Connection with Mainstream and non funded Community Supports
- Plan Budget oversight, audit and management
- Regular Plan Support Meetings and Reviews.
SPECIALIST SUPPORT COORDINATION (SSC)
Requires an intensive and specialised service response to manage and mitigate the complexities of an individuals stated needs and goals.
H&CS Support Coordinators have high level expertise and extensive experience in Psychosocial Disability, Complex Stakeholder engagement and management, ASD, Clinical Care Requirements and training responses, Housing, Criminal Justice interventions and complex presentations.
PSYCHOSOCIAL RECOVERY COACHING
Psychosocial Recovery Coaches (PRC) support participants with psychosocial disabilities to live a full life.
PRC work collaboratively with participants, their families, carers and other services to design, plan and implement a Recovery Plan, and assist with the coordination of NDIS and other supports.
‘Participants will have the option of selecting a recovery coach with lived experience or a recovery coach with learnt knowledge of psychosocial disability and mental health’ (Source – NDIS).
The H&CS PRC’s are trained, experienced, supervised and supported Mental Health professionals, whom draw on their extensive experience of disability as practitioners to provide coordinated and structured supports to their clients.
